La storia prende spunto dagli esperimenti del dottor Josef Mengele, il famigerato scienziato del Terzo Reich che, dopo avere compiuto atroci esperimenti nei lager nazisti, prosegue, durante la latitanza in Brasile, la sua operazione di "pulizia" della razza ariana.

Il film è ispirato a fatti realmente accaduti e racconta la storia del criminale di guerra tedesco Willi Herold, che rubando l'identità di un ufficiale tedesco, ha orchestrato l'uccisione di disertori e altri prigionieri in uno dei campi di Emslandlager.
