Dopo che - nelle vicende narrate nel secondo episodio della serie The Eternal Story, inedito in sala in Italia - Madoka si era sacrificata, pur di eliminare le Streghe e riscrivere le Leggi dell'Universo, nella città di Mitakihara sembra che nulla di tutto ciò sia mai accaduto e le ragazze magiche si ritrovano nuovamente insieme a sconfiggere i malefici Nightmares. Ma Homura, l'unica che sembra ricordare cosa sia avvenuto in realtà a Madoka, non è convinta di ciò che vede e decide di scoprire se la realtà in cui vivono le ragazze sia effettivamente tale o un'illusione magica.

Una misteriosa entità aliena si sta dirigendo verso la Terra, con l’intenzione di congelarla completamente. Ma un cristallo utile a questo scopo viene perso e si stacca dalla cometa principale. Il cristallo viene ritrovato da Kakeru Ozora, un giovane astronomo di salute cagionevole, diventato amico della gattina Luna, dopo averle salvato la vita da un incidente d’auto. La vicinanza al cristallo fa precipitare ulteriormente le condizioni del giovane, e Luna decide di aiutarlo con l’aiuto delle Guerriere Sailor che intanto stanno avendo il loro da fare con le terribili Snow Dancers, che hanno il compito di congelare la terra. Tale entità è proprio la cometa osservata da Kakeru, la Princess Snow Kaguya, già respinta una volta al tempo del Silver Millennium dal potere del Cristallo d’Argento.

In occasione delle vacanze invernali, una fortunata vincita porta Sakura e i suoi amici nella misteriosa Hong Kong. Qui il gruppo farà la conoscenza della antica famiglia del piccolo Shaoran Li. ma dovrà anche affrontare una nuova sfida e scontrarsi con la Madoushi, una veggente che era stata respinta da Clow Reed del quale era innamorata.

Il film è ambientato quattro mesi dopo l'ultima puntata dell'anime. Inizia con la demolizione della vecchia casa di Eriol, e una carta nascosta nelle fondamenta, appare. Intanto, Sakura, dopo aver trasformato tutte le carte di Clow in carte di Sakura, è a tutti gli effetti la nuova padrona delle carte, nonché un'eccellente maga. La ragazza si sta preparando al Nadeshiko Festival, un festival che si tiene a Tomoeda, in cui dovrà recitare il ruolo della principessa in una commedia organizzata dalla sua classe. Nel frattempo, un nuovo parco di divertimenti viene aperto a Tomoeda, sul terreno su cui era costruita la casa di Eriol. Quando Sakura e Tomoyo si recano al parco, Sakura avverte una forza molto forte, proveniente dal parco...
