本作は『ソウ』(04)と『ソウ２』(05)の間に起こった出来事を描く一作。前作『スパイラル：ソウ オールリセット』(21)にはシリーズのアイコンである殺人鬼“ジグソウ”が登場しなかったが、今作ではファンの期待に応えて（？）シリーズに復活。 末期がんで余命わずかのジョン・クレイマー（ジグソウ）が実験的な治療を受けるためにメキシコに向かうが、その治療が詐欺だと知り、詐欺師や加担した医師たちに死のゲームを仕掛けていく。
