Den gamle fengselsøya Alcatraz (The Rock) okkuperes av elitesoldater ledet av oberst Francis X. Hummel. De bruker turister som gisler og stiller inn raketter med biologiske stridshoder mot San Fransisco. Mannen som kan redde byens innbyggere fra en sikker død er John Patrick Mason, den eneste fangen som har rømt fra Alcatraz og overlevd. Med seg har han FBI-agent Stanley Goodspeed.
